Add clinic: “Primary Care Transition Clinic.” Enter your province (or other option) and health card or other identification. Select from our list of visit reasons and request an appointment date and time. Once your appointment is confirmed, you will receive a virtual link 5 minutes prior to the scheduled visit. We will call you if there are any difficulties connecting virtually.
Pay after your appointment once your invoice is received by email.

Book and manage your appointments, send and receive messages from Primary Care Transition Clinic (PCTC) and attend virtual appointments with your NP or physician directly through the app.
