On your phone, download the CHR Connect App using App store or Google Play .
(You can also use CHR Connect on your desk top)
Add clinic: “Primary Care Transition Clinic.” Enter your province (or other option) and health card or other identification. Select from our list of visit reasons and request an appointment date and time. Once your appointment is confirmed, you will receive a virtual link 5 minutes prior to the scheduled visit. We will call you if there are any difficulties connecting virtually.
Pay after your appointment once your invoice is received by email.
